Treatment for Excessive Sweating
Excessive sweating is a common problem amongst  both male and female and is technically termed as hyperhidrosis. The sweating can be on various parts of the body but tends to be common on the fair and hairline area, groin and primarily the underarms. Sweating is a completely natural body function that occurs to regulate body temperature but when it is  excessive it is termed as hyperhidrosis.

Cause of Excessive Sweating
Hyperhidrosis can be classified dependent on the time of onset and can be either congenital or acquired. Focal hyperhidrosis is found to commence during adolescence or in some cases before and seems to be an inherited genetic trait. Secondary hyperhidrosis, which can start at any point in life. This can be due to a disorder of the thyroid or pituitary glands, diabetes, tumours, menopause or certain drugs.


Surgical Options

Laser Sweat Ablation provides a long term permanent improvement
Laser sweat ablation works by destroying and removing the eccrine glands in the affected area and is generally used to treat the underarms. It provides a permanent solution to hyperhidrosis.

The procedure can be carried out in a normal clinic by a doctor or cosmetic surgeon.In order to determine the areas to be treated a sweat test is first carried out which determines the areas of the underarms that require the most treatment. These areas are then marked before the patient is injected with a local anaesthetic to numb the area for treatment. Small incisions, approximately 2 to 3 millimetres in length are created in the underarms and the skin is separated from the underlying layer of fat. An infrared laser fibre is then inserted  into the incisions and discharged, destroying the eccrine glands that are present in the underside area of the skin.

Once the glands are destroyed, the doctor then removes the dead tissue by use of a suction before the underarm skin is arranged back in place. The incisions are not large enough to require stitches so a compression bandage is applied to the area to ensure correct healing. The entire procedure usually takes 1-2 hours.

Recovery from Laser Sweat Ablation
Pain, bruising and numbness may occur after laser sweat ablation but this is temporary. There is also a risk of loss of pigmentation in the area. The doctor often prescribes antibiotics to prevent any infection from occurring.

Vaser Sweat Ablation provides a long term permanent improvement
Vaser Sweat Ablation is a newer treatment and performed whilst the patient is under a light sedation. It provides a permanent solution to hyperhidrosis utilising ultrasound technology.
The process for laser sweat ablation is a very simple one with the doctor first injecting a tumescent fluid into the underarm area of the patient: this will allow the ultrasonic waves to pass through the liquid more readily that through a solid material. The ultrasonic waves are set at a specific frequency to kill the sweat gladdens without damage to the surrounding tissue in the area. The dead tissue is suctioned out of the area with one stitch only required to close the area. The procedure takes around 30 minutes and the sweat is moderated with minimal downtime to the patient.

As with Laser Sweat Ablation there may be some swelling and bruising to the area which will dissipate within 5-7 days. Antibiotics may be prescribed by the doctor to aid with wound healing


Non-Surgical Options

Anti Sweating Injections
Anti-wrinkle injections, normally utilised to help reduce facial wrinkles can also be used to stop sweating. Anti-wrinkle injections are commonly used  for the treatment of hyperhidrosis, specifically in the underarms, face and scalp areas. It can also be used in the palms of the hands and the soles of the feet, but is painful to inject in this area and will require some form of sedation . The underarms and scalp area can be treated using a local anaesthetic cream.
The Anti-wrinkle injections work by blocking the nerve signal to the sweat glands in the area it is injected into. Commonly 100 units  are used for the underarm area.

After application of anaesthetic cream, a starch-iodine test is usually performed. This involves putting iodine on the area to be treated and allowing the patient to sweat for short period of time. Starch is then placed on the area and the sweaty areas highlight in black and identified for treatment with the injections

The anti wrinkle injection is then injected superficially into the area with numerous small injections. The process takes around 30-60 minutes depending upon the size of the area requiring treatment. The effects take around 3-10 days to be noticed with full effects within 2 weeks. Ant wrinkle injections last for between 8 weeks to 4 months depending upon the client and then must be repeated.
There is usually no down time with anti-wrinkle injections and patients can return to normal life immediately following treatment.

The procedure for injecting a dermal or cosmetic filler product is relatively simple and painless. Dependent upon the type of filler to be used a skin test may be required to ensure the patient does not have an allergic reaction to the product, but in most cases this is not required.

An anaesthetic cream may be applied to the area for treatment 20 minutes or so prior to the procedure. The treatment is delivered using a pre filled syringe equipped with a very fine needle which is injected into the dermal layer of the skin. The deeper the hollow, the deeper the product is injected into the skin with a thicker filler product used to shape the chin. Different injection techniques may be used depending on the type of filler and the area being treated but usually consist of either a linear threading technique where the needle is inserted into the middle of the wrinkle or a serial puncture technique where several injections are inserted along the wrinkle.





Dermal Filler Recovery
The majority of patients can return to normal life immediately following a dermal or cosmetic filler treatment to the chin area. The effects can be seen immediately and depending upon the area treated and the product used, results can last for between six and ten months. There may be a small amount of swelling or bruising at the site of the injection which will generally settle down over the following 48 hours with patients recommended not to touch the treated area. Some patients may feel the requirement to use a mild painkiller containing paracetamol for a few days following treatment

The ageing of facial skin is ongoing and commences towards the age of 20, progressively getting worse unfortunately. During the mid 30’s and early 40’s tends to be the age where most people being with minor cosmetic procedures such as anti wrinkle injections and fille

The 20s
in the teen years and before most people enjoy a skin with good hydration, excellent repair capabilities, smooth and even texture and a low to moderate oil flow. During the 20’s we start to see small changes in the skin including the below:

Pore size begins to increase
Cell turn over increases
Mild drop in skin elasticity with skin very slowly starting to lose some of its collagen and elastin fibres.
Oiliness or dryness can increase
sun spots may begin to be more evident
Texture of the skin may begin to change

Skin care in 20s
It is important to begin looking after skin well in the 20’s to delay the ageing process and avoid damage from factors such as the sun. Sunscreen in the morning is a must with a Retinoid (Vitamin A) cream at night to prevent breakouts and pigmentation.
A day time moisturiser will aid with skin repair.

Ingredients to identify in 20s skin care include
ZINC OXIDE SUNCREEN - this provides both UVA and UVN protection against pigmentation, skin damage and skin cancers.
RETINOIDS

The 30s

Skin care in 30s
Sunscreen, moisturisers and retinoids should all be continued with the addition of anti oxidant ingredients such as Vitamin C and fruit acids. Antioxidants are an effective way externally and internally to treat skin ageing in the 30’s.

Ingredients to identify in 30s skin care include
Zinc oxide sunscreen
Retinoid creams.
Vitamin C (L-Ascorbic acid, L-ascorbate) which is the most popular of topical antioxidants. The Vitamin C is essential for collagen production and UV protection in the skin. This must be stable, non irritating and acidic to be effective.
Fruit acids or Alpha Hydroxy Acids- These include lactic acid, citric acids and magic acid. They must have a concentration of greater than 8% to be effective and can be found in skin care products such as cleansers, exfoliants and moisturisers.



The 40s
During the 40’s skin begins to age at a more rapid rate and more of the following can be identified:

Decreased in collagen, hylauronic acid and elastin resulting in sagging skin
Reduced cell turnover resulting in rougher skin
Decrease in immune and dermal repair
More prominent frown, forehead and facial lines
Loss of volume in mid face
Effects of sun damage may be seen on the skin
Increase in skin capillaries and reddening

Skin care in the 40s
The same skin care for the 30’s should be used with the addition of a product containing a Niacinamide. A Niacinamide is an ingredient which provides immune protection.


It is important to use good skin care in order to slow down the signs of ageing. This can also be done by avoiding factors such as sun, smoking and excess alcohol intake. Laser treatments, cosmetic fillers, dermal rolling and good skin care all can improve the appearance of skin at any age.

A Chemical Peel uses a varying strength chemical solution applied to the skin to remove dead skin cells and stimulate the production of new cells. A chemical peel occurs when a dermatology safe solution is applied to the skin to remove some of the outer layers and reveal a fresh new layer of skin cells underneath, effectively tightening the skin and restoring a more youthful appearance. The Chemical Peel Procedure
Superficial/ Glycolic Chemical Peel
A superficial or glycolic chemical peel is applied to the skin and left on between three and ten minutes before being rinsed off and neutralized. A mild stinging sensation may be felt on the skin with pinkness and blotching which will fade after one to two hours. A faint discoloration may also occur but otherwise there are no after effects and normal daily life can be resumed straight away. A superficial chemical peel treatment should be scheduled weekly for a time period of four to six weeks with a recommendation of monthly peels for skin maintenance. A mild or medium depth chemical peel can be combined with microdermabrasion, invigorating cell renewal to create a healthy, balanced skin surface

Medium-depth/TCA Cosmetic Peel
The skin requires treatment for two weeks prior to the chemical peel, with retin A cream and hydroquinone cream which primes the skin and helps prevent skin darkening after the peel. Following cleansing the chemical peel solution is applied evenly to the skin with the treated area turning a whitish grey over several minutes. To neutralize the acid in the peel, cool saline compresses are then applied. The face may swell during application with stinging or burning affecting the skin for up to an hour following treatment. The healing process will take around a week in which the skin will begin to itch and peel, leaving a bright red new layer of skin which will fade to a fresh pink over the next four to six weeks. The treatment should be repeated monthly dependent upon the results required and dermatologist recommendations with a maintenance program of a cosmetic peel every 6-12 months.

Deep/Phenol Chemical Peel
A deep chemical peel requires some form of anesthesia either oral or intravenous depending upon the extent of the procedure and must be performed in an operating theatre to monitor the heart rhythm The skin is cleansed and a strong peel applied and left on for around 30 minutes before being neutralized. The face swells in the first two days and can be very itchy during the healing process, taking around two weeks before the face is healed with redness persisting for 2-3 months. One treatment can produce dramatic results with a deeper chemical peel but healing time can be lengthy.

Chemical Peel Risks & Recovery
Although uncommon, these complications can occur with the application of chemical peels:
Excessive redness and peeling of the skin with deeper peels– This can be covered with   special make up
Infection – This is very rare and can be treated with antibiotics
Hypopigmentation -  lightening of the skin color
Hyper pigmentation - darkening of the skin color
Cold Sores – These can be stirred up during chemical peels and cold sore cream is recommended before and after a cosmetic peel procedure

A chemical peel may not be suitable for a dark, olive or Asian skin but this can be determined by a doctor with a simple test. Clients with facial melanomas are not suitable for any chemical peel treatment and deeper peels cannot be performed on a client with a cardiac arrhythmic history.

Cost of Chemical Peels
The cost of Chemical peels will vary between dermatologist and state with a price range of approximately $600-$900. Light or mild chemical peels can cost as little as $150 with deeper, more extensive peels as much as $5000 for a complete treatment program at a reputable clinic.
